Grants Competition Documentation for International Students

The following forms must be submitted before you can receive your Student Research Grant. All forms should be given to Assistant Dean LaRuth McAfee in Room 229 of Bascom Hall during business hours (M-F, 8:00 a.m-4:30 p.m.). If she is unavailable, the documents may be left at the main office of the Graduate School (Room 217).

If you are a Resident Alien for tax purposes, you do NOT need to submit this documentation.

Documentation for J-1 International Students

  • Passport picture page (photocopy)
  • Passport page with Homeland Security Stamp of most recent entry date listed on I-94 (photocopy)
  • W-8BEN form (ORIGINAL, not photocopy. Complete items # 1, 3, 4 [home country address], 5 [Madison address], and 6.  Check #9a and fill in your home country, then sign the bottom.)
  • I-94 (To retrieve this electronically, go to https://i94.cbp.dhs.gov/I94/request.html, enter the appropriate information, and click submit.  Please print the resulting page, which will show your I-94 Admission Record Number.)
  • DS-2019 (photocopy)
  • ONE of the following items:
    • An application form or acceptance letter for a Social Security Number, signed or stamped by the appropriate Acceptance Agent, or your Social Security Number
      OR
    • An application form or acceptance letter for an Individual Taxpayer Identification Number (ITIN) signed or stamped by the appropriate Acceptance Agent or your Individual Taxpayer Identification Number.

If you do not have a SS#/ITIN, you will be required to apply for one if you receive an award.

Documentation for F-1 International Students

  • Passport picture page (photocopy)
